Colleen Host- Principal Project Officer- Service Deliver & Implementation (SDI)- Home Stretch and Leaving Care Reform- Colleen is driven by her passion to work toward systemic change for young people who are transitioning from the care system into adulthood. Colleen has broad and significant experience in the leaving care sector as: a Social Worker; serving over 20 years in the Mirrabooka Child Protection District in Leaving and Post Care; with Anglicare Home Stretch Community of Practice during Home Stretch implementation; and now actively working in the SDI project team alongside the districts, Home Stretch service providers and Lived Experience consultants to implement Home Stretch and leaving care continuum reforms. Colleen remains focused on working to ensure best outcomes for young people with a care experience.
